The 15 minute Health Check includes: - Self Assessment Questionnaire - Blood Pressure check - Cholesterol (Total/ - Glucose check - Waist Measurement - Type II Diabetes Risk Assessment - Cardiovascular Disease Risk Assessment (for those over 45 years of age) - Information sheets - Individual results booklet - Personal consultation - Company statistical report (for organisations booking 50 or more Health Checks)

Workplace Health Promotion Grants (Funding for workplace health programs) Workplace health promotion grants are one-off funding opportunities for employers to build or expand programs that promote worker health and encourage healthy behaviours in the workplace. Health by Design can assist employers with their application for the grant and provide corporate wellness solutions that meet the grant guidelines. The WorkHealth program will fund workplace health promotion activities that: - encourage healthy behaviours as well as improve the health and well-being of people at work; - create a supportive work environment that makes healthy choices easy for workers and encourages them to maintain healthy behaviours in the future. To be eligible to apply for a workplace grant you must: - Have offered health checks to the majority of your workers in the past two years - Be a Victorian business or have Victorian based operations - Pay a WorkSafe Injury Insurance premium or be an approved WorkSafe self-insurer Successful workplace health promotions programs are designed around the needs of workers and the goals of the business - so no two programs will be the same.
